The Project BOOST Mentoring Program: What Does It
Offer?Participating BOOST sites will take advantage
of a program specifically tailored to their unique needs that
includes: 1 year of individualized
mentoring by national physician experts in quality improvement with
demonstrated accomplishment in care transitions at their own
institutions Development of action /
implementation plans 6-8 Mentor / Site
leader telephone calls, e-mail communications Site visit by mentor to meet with BOOST team, site leaders,
ancillary teams, system-wide presentation (such as grand
rounds)
The BOOST toolkit
providing both clinical interventions and practical step-wise project
management tools Face to face two day
training session for QI teams (up to 5 participants) to learn about the
intervention, exchange ideas with other BOOST sites and work with their
mentor to establish an action plan Follow-up online webinar Participation in the web-based BOOST collaborative where
participants share ideas, documents and other resources via an active
listserv, document sharing and other web-based tools. Quarterly
teleconference with site to site Q & A Teach-Back training, video and curriculum for local use to
train clinicians to improve communication with
patients Access to online Data Center to
collect and analyze data, benchmark performance against comparison
groups at your institution and other BOOST facilities
Why is Tuition Discounted for CA Sites?
CA BOOST sites are subsidized through two grants: 1- California
Healthcare Foundation (CHCF) and 2- California HealthCare Foundation
Hospital Association of Southern California (LA Care Health Plan).
The CHCF grant provides discounted tuition for 20 sites in the state
and establishes a state-wise Leadership Council to coordinate efforts
throughout the state to reduce avoidable readmissions, and train
additional mentors to sustain and spread project BOOST beyond the two
year grant period.
The LaCare grant provides discounted tuition for up to 10 sites in
LA County. In addition to BOOST training, tools, and mentoring,
these sites will also join the Readmissions Reduction
Collaborative. A dedicated QI Specialist will work on site with
teams to facilitate implementation.
How Many Sites are Accepted?Participation is limited to up to 20
sites. When is the

2012. When does Training Begin?
Accepted programs will begin training in March 2012.

Tuition?The subsidized CA site tuition contribution
