Palliative Care for Patients With Heart Failure
This CME activity is intended for clinicians caring for patients with
heart failure (HF) across the care continuum. They may include
hospitalists, cardiologists, other physician specialists, and
nonphysician clinicians such as advance practice nurses who are involved
in the treatment and management of patients with advanced HF who may
benefit from palliative care. On completion of this offering,
participants will be able to:

Recognize that a broad spectrum of patients with HF can benefit
from palliative care

Describe effective dyspnea management, as well as pain and symptom
control, for patients with HF

Recognize the value of implementing early discharge plans,

importance of ascertaining patients’ goals when developing a
treatment plan
