CMEs for Hospitalists

In an effort to provide our members with the numerous opportunities to gain continuing medical education (CME) relevant to hospital medicine, leaders of the hospital medicine community have lent their expertise to develop online CME modules. Our current offering of SHM developed modules as well as a listing of peer reviewed online accessible CME modules around various topics for healthcare professionals who specialize in hospital medicine are featured below.

Online CME Listings

Team Communications for the Management of the Hospitalized Patient with Heart Failure
This CME activity is intended for clinicians caring for patients with heart failure (HF) across the care continuum. They may include hospitalists, cardiologists, other physician specialists, and nonphysician clinicians such as advance practice nurses who are involved in the treatment and management of patients with advanced HF who may benefit from palliative care.
On completion of this offering, participants will be able to:

  • Identify three communication issues related to heart failure (HF) management.>
  • Define strategies to improve communication within the team and with the patient and the family.
  • List three areas of self-management that must be reviewed with the patient prior to discharge from hospital.

This case is worth 1 AMA PRA Category 1 Credits™ & 1 nursing contact hours.

Optimizing the Heart Failure Discharge Transition
This CME activity is intended for hospitalists, cardiologists, primary care providers, other physicians, and non-physician clinicians such as advance practice nurses specializing in the management of patients with heart failure.  Upon completion of this educational offering, participants will be able to:

  • Employ a multidisciplinary approach to the care of patients with heart failure that begins at hospital admission and continues through all care transitions
  • Accurately and effectively reconcile medications across the care continuum
  • Recognize the value of initiating discharge planning early in the course of the hospitalization, including communication with outpatient clinicians
  • Recognize the importance of post-discharge follow-up
  • Effectively communicate with patients and families critical elements of successful self-management, especially in the post-discharge period
  • Identify specific issues that increase the risk of care transitions for patients with heart failure

Johns Hopkins Consultative Medicine Essentials (JHCME) for Hospitalists
This online consultative medicine resource created and designed for hospitalists by hospitalists, will provide authoritative and comprehensive training in general medical consultation for hospitalized patients through eight accredited case-based modules.  Topics include (but are not limited to):

  • Perioperative Cardiac Risk Assessment and Management
  • Diagnosis and Management of Acute Mental Status Changes: Delirium
  • Perioperative Acute Renal Failure Diagnosis and Management
  • Pulmonary Risk Management
  • Perioperative Management of Anticoagulation
  • Venous Thromboembolism Prophylaxis
  • The Role of the Medical Consultant

Each module includes its own set of objectives and knowledge assessment activities. Three more modules will be available by the end of 2008 and another eight new modules will be available by the end of 2009. Please contact the course director, Dr. Lenny Feldman, at LF@jhmi.edu with questions.

Prevention of Venous Thromboembolism in Medical Patients

  • Perform risk stratification for the development of venous thromboembolism (VTE) in the hospital setting
  • Describe evidence-based prophylactic strategies to prevent VTE in high-risk medical patients
  • Initiate a quality improvement project to address underutilization of preventative measures aimed at predictable complications of illness
