Session Description: This session will address
proposed changes in how hospitals are reimbursed for readmissions. The
discussion will initially be framed by providing historical perspective
on hospital reimbursement. Then the presenter will review what is known
about the frequency of hospital readmissions. Readmissions will be
reviewed within the general context of patterns of post-hospital care.
Next the perspective of national and federal quality improvement
entities will be reviewed. Highlights from a recent consensus document
authored by 6 physician professional societies will then be referenced
to provide a framework for discussion accountability. The session will
conclude with an update of proposed changes in national health
policy.
Objectives:
Upon completion of this session, participants should be able to:
- Participants will understand the frequency of post-hospital
readmissions
- Discuss proposed changes in reimbursement as articulated by the
Medicare Payment Advisory Commission (MedPAC)
- Consider the impact of proposed changes to national reimbursement
policy on the operations and workflow of hospital clinicians

Session Description: Hospital discharge can be a
dangerous process for patients. Research documents significant patient
safety and quality deficiencies in our current system. Potentially 1 in
5 patients suffers an adverse event after discharge, predominantly due
to medications. Of note, up to two-thirds of these adverse events could
be avoided or mitigated, primarily through improved communication. Many
patients leave the hospital with test results pending, and discharge
summaries, even if completed in a timely manner, tend to lack critical
information such as diagnostic test results, treatment or hospital
course, and follow-up plans. The Society of Hospital Medicine in
collaboration with the American College of Physicians, Society of
General Internal Medicine, the American Board of Internal Medicine and
the American Medical Association has led efforts to enhance the
discharge transition process. With funding from The John A. Hartford
Foundation, Project BOOST (Better Outcomes for Older adults through Safe
Transitions) implemented a toolkit at 30 hospitals across the U.S. to
improve the hospital discharge transition.

Session Description: Join us for this interactive
session that will detail the CPT codes you use on a daily basis and
address your most common documentation and coding questions. We will
discuss all aspects of coding for the patient's entire hospital stay,
including admissions, daily visits and discharges. The importance of
choosing the right category of codes (inpatient versus observation) will
be discussed. Learn how to properly document when coding by time, both
for daily visits and discharges. Critical care and prolonged services
will be covered, and the latest information on consultations and
transfers of care will be presented. Medicare has proposed to do away
with consultation codes in 2010; when the Final Rule is published later
this year, we will have instructions on how to appropriately code when
you are asked to provide consultative services, including preoperative
evaluations.
