Session Description: Join SHM's interactive session on co-management and how today's hospitalists have an increasing role in the medical management of surgical patients. While there are opportunities for hospitalists to add real value as co-managers of surgical patients, the general definition of co-management is vague and varies noticeably from one hospital to another.

Over the past year, SHM has formed both a Task Force and Advisory Panel to help figure out what the "must-have" ingredients are for a successful co-management program. Even though there are variations on the way co-management programs have been established and operate, there is consensus about the factors common to the most effective programs. This webinar will discuss strategies and key ingredients for building a successful program.

Audience:

The primary audience is Hospitalists. It is also appropriate for Nurse Practitioners and Physician Assistants in Hospital Medicine and Surgeons.

Objectives:

Upon completion of this session, participants should be able to:

  • Discuss potential challenges, pitfalls, and ways to avoid them when developing your co-management program
  • Identify metrics to assist you in measuring the effectiveness of your co-management program
  • Describe essential components necessary for developing an effective co-management program

Session Description: This session will present the Teach Back communication method as a means of improving patient understanding of self-care instructions and overview a curriculum for trainers to use locally.

The session will discuss barriers patients face in understanding and participating in their post-discharge care plans. A video will demonstrate specific techniques to be used to improve communication between clinicians and providers. Additionally, the presenters will discuss strategies to evaluate the effectiveness of current clinician communication with patients; implement training programs for clinical staff and evaluate the adoption of the improved communication strategy.

Audience:

The target audience for this program is all hospital based clinicians, specifically those educating patients about self-care instructions during the hospital stay or at the time of discharge including medication use, outpatient follow-up, dietary and lifestyle changes and limitations.

Objectives:

Upon completion of this session, participants should be able to:

  • List common reasons patients do not recall or misinterpret self-care instructions and hospital discharge care plans
  • Describe the Teach Back process and how it can be used to identify with patients with inadequate health literacy
  • Explain how the Teach Back process facilitates patient comprehension of care plans
  • Adopt the Teach Back method to educate patients in the hospital

Session Description: This session will address proposed changes in how hospitals are reimbursed for readmissions. The discussion will provide a historical perspective on hospital reimbursement and review the literature on demographics and frequency of hospital readmissions. Opportunities to prevent readmissions will be reviewed within the general context of post-hospital follow-up and care.

Session Description: Hospital discharge can be a dangerous process for patients. Research documents significant patient safety and quality deficiencies in our current system. Potentially 1 in 5 patients suffers an adverse event after discharge, predominantly due to medications. Of note, up to two-thirds of these adverse events could be avoided or mitigated, primarily through improved communication. Many patients leave the hospital with test results pending, and discharge summaries, even if completed in a timely manner, tend to lack critical information such as diagnostic test results, treatment or hospital course, and follow-up plans. The Society of Hospital Medicine in collaboration with the American College of Physicians, Society of General Internal Medicine, the American Board of Internal Medicine and the American Medical Association has led efforts to enhance the discharge transition process. With funding from The John A. Hartford Foundation, Project BOOST (Better Outcomes for Older adults through Safe Transitions) implemented a toolkit at 30 hospitals across the U.S. to improve the hospital discharge transition.
