CO-MANAGEMENT OF
THE HOSPITALIZED PATIENT: SHARED RESPONSIBILITY, AUTHORITY AND
ACCOUNTABILITYThursday, October 28, 2010
2:00 - 3:30 PM EDT, 1:00 - 2:30 PM CDT,
12:00 - 1:30 PM MDT, 11:00 AM - 12:30 PM PDT
Presented
by:
Michael S. Pinzur, MD Professor
of Orthopaedic Survey & Rehabilitation, Loyola University Medical
Center Hugo Q. Cheng, MD
Director of Neurosurgery Co-Management Service, University of California
San Francisco Session Description: Join SHM's
interactive session on co-management and how today's hospitalists have
an increasing role in the medical management of surgical patients. While
there are opportunities for hospitalists to add real value as
co-managers of surgical patients, the general definition of
co-management is vague and varies noticeably from one hospital to
another. Over the past year, SHM has formed both a Task Force and
Advisory Panel to help figure out what the "must-have" ingredients are
for a successful co-management program. Even though there are variations
on the way co-management programs have been established and operate,
there is consensus about the factors common to the most effective
programs. This webinar will discuss strategies and key ingredients for
building a successful
program. Audience: The primary audience is
Hospitalists. It is also appropriate for Nurse Practitioners and
Physician Assistants in Hospital Medicine and
Surgeons. Objectives: Upon completion of
this session, participants should be able to: - Discuss
potential challenges, pitfalls, and ways to avoid them when developing
your co-management program
- Identify metrics to assist you in
measuring the effectiveness of your co-management
program
- Describe essential components necessary for developing
an effective co-management
program

Description: This session will present the Teach Back
communication method as a means of improving patient understanding of
self-care instructions and overview a curriculum for trainers to use
locally. The session will discuss barriers patients face in
understanding and participating in their post-discharge care plans. A
video will demonstrate specific techniques to be used to improve
communication between clinicians and providers. Additionally, the
presenters will discuss strategies to evaluate the effectiveness of
current clinician communication with patients; implement training
programs for clinical staff and evaluate the adoption of the improved
communication strategy. Audience: The
target audience for this program is all hospital based clinicians,
specifically those educating patients about self-care instructions
during the hospital stay or at the time of discharge including
medication use, outpatient follow-up, dietary and lifestyle changes and
limitations. Objectives: Upon completion of
this session, participants should be able to: - List common
reasons patients do not recall or misinterpret self-care instructions
and hospital discharge care plans
- Describe the Teach Back
process and how it can be used to identify with patients with inadequate
health literacy
- Explain how the Teach Back process facilitates
patient comprehension of care plans
- Adopt the Teach Back method
to educate patients in the
hospital

Description: This session will address proposed changes in how
hospitals are reimbursed for readmissions. The discussion will provide a
historical perspective on hospital reimbursement and review the
literature on demographics and frequency of hospital readmissions.
Opportunities to prevent readmissions will be reviewed within the
general context of post-hospital follow-up and care. The session
will also review national quality improvement initiatives and current
and pending incentives and penalties related to readmissions and care
coordination. Highlights from a recent consensus document authored by 6
physician professional societies will be referenced to provide a
framework
BOOSTing the Discharge Transition

Description: Hospital discharge can be a dangerous process for
patients. Research documents significant patient safety and quality
deficiencies in our current system. Potentially 1 in 5 patients suffers
an adverse event after discharge, predominantly due to medications. Of
note, up to two-thirds of these adverse events could be avoided or
mitigated, primarily through improved communication. Many patients leave
the hospital with test results pending, and discharge summaries, even if
completed in a timely manner, tend to lack critical information such as
diagnostic test results, treatment or hospital course, and follow-up
plans. The Society of Hospital Medicine in collaboration with the
American College of Physicians, Society of General Internal Medicine,
the American Board of Internal Medicine and the American Medical
Association has led efforts to enhance the discharge transition process.
With funding from The John A. Hartford Foundation, Project BOOST (Better
Outcomes for Older adults through Safe Transitions) implemented a
toolkit at 30 hospitals across the U.S. to improve the hospital
discharge transition.

Description: Join us for this interactive session that will
detail the CPT codes you use on a daily basis and address your most
common documentation and coding questions. We will discuss all aspects
of coding for the patient's entire hospital stay, including admissions,
daily visits and discharges. The importance of choosing the right
category of codes (inpatient versus observation) will be discussed.
Learn how to properly document when coding by time, both for daily
visits and discharges. Critical care and prolonged services will be
covered, and the latest information on consultations and transfers of
care will be presented. Medicare has proposed to do away with
consultation codes in 2010; when the Final Rule is published later this
year, we will have instructions on how to appropriately code when you
are asked to provide consultative services, including preoperative
evaluations. |

Hospitalist Staffing and SchedulingA
collaborative webinar from: About the WebinarThe need
to provide 24 hour coverage 7 days a week, while at the same time
ensuring reasonable continuity between hospital to patient, and
reasonable physician lifestyle, presents many challenges. In this
session the presenters will review data regarding current hospitalist

staffing ratios and workloads so you can compare them to the same
figures for your own practice;
- Discover popular hospitalist
scheduling models and how they have changed over time;
and
- Recognize the pros and cons to various scheduling
approaches, including effects on things such as efficiency, costs,

Are you ready for the new HIPAA
privacy and security mandates? The American Recovery and
Reinvestment Act (ARRA) greatly expanded the privacy and security
requirements for practices under HIPAA. New mandates will force
practices to adopt policies and procedures in order to avoid new
enforcement provisions and significantly increased penalties. Critical
changes include: onerous new provisions for accounting of disclosures;
new patient rights that you will need to incorporate into your policies
and staff training; new requirements should your patient data be
breached; modifications that will need to be made to your business
associate relationships; and increased penalties up to $50,000 per
violation to a maximum of $1.5 million a year in the most egregious
cases of data breaches. One of the nation's leading privacy experts will
lead you through these changes and how they will impact your
practice. Following this session, you will be able
to: - Identify the new requirements placed on
your practice by the ARRA statute and related
regulations;
- Understand the timeline for the government's
implementation of these requirements; and
- Recognize the steps
your practice will need to take to prepare for these changes and avoid
government enforcement action.
