Hospitalist Orthopedist Patient Service (HOPS) Co-Management Program
SHM has accepted 10 sites to participate in the HOPS Program. Hospitalist mentors with experience in Co-Management programs and QI expertise will work with participating teams to tackle site-specific issues in developing a Hospitalist/Orthopedic surgery Co-Management program. Key issues and guidance provided by the site mentor will include:
- Developing a service agreement
- Determining roles and responsibilities of key stakeholders
- Addressing financial and payment issues
- Developing key metrics to measure program effectiveness, and
- Obtaining buy-in from physicians and hospital administration
Mentoring will occur during six scheduled telephone calls offered over a six month period. Guidance will be organized around the Co-Management Program Implementation Guide, SHM’s step-by-step guide for building a successful co-management program for surgical patients.
- Each of the selected sites will receive six months of mentoring from national experts to help start a co-management program at your hospital. There is no fee for participation. We encourage you to take advantage of this incredible opportunity. Selected sites will receive:
- Six months of mentoring provided via teleconferences, e-mails, and webinars tailored to the unique needs of your institution.
- Project management tools.
- Evaluation tools.
- Reference articles and materials from other established co-management programs.
- Access to networking opportunities to share successes and challenges with peer sites.

More About the HOPS Co-Management Program
Goal and outcomes: The goal of the HOPS Co-Management Program is to assist participating sites in developing and implementing a comprehensive and well-planned co-management program. By implementing an effective co-management program, this project aims to support sites in optimizing the care of orthopedic surgery patient and enhance communication between members of the care team.
APPROACH:
- Implement best practices: The Co-Management Advisory Panel comprises a multi-disciplinary team of experts in the fields of hospital medicine, rehabilitation medicine, orthopedic surgery, nursing, and hospital administration. In addition to their considerable knowledge and contributions, a comprehensive literature review and interviews with existing programs was conducted to determine best practices - leading to the development of the Co-Management Program Implementation Guide.
- Utilize resources to implement best practices. The following materials will be available to site participants:
- Co-Management Program Implementation Guide
- Data Collection Tools
- Project Management Tools
- Improvement Project Tools
- Review of key literature
- Exchange Information and Share Success stories
- Provide technical support. The HOPS Co-Management Program offers technical support including the six month mentoring program sponsored by Ortho-McNeil. The selection of mentors and formulary choices are independent of sponsorship.
