Yuma Regional Medical Center Photographic Wound Documentation Form
This tool can be found on IHI.org -provided by the Yuma Regional Medical Center. This documentation form is used to record several aspects of a wound and its surrounding skin. This is done at intervals for the same wound which will show the healthcare provider if the wound is healing or not. It is also used to document pictures of the wound after treatments.
