Williams, MD members of Project BOOST (Better Outcomes for Older Adults in Safe Transitions) at the Society of Hospital Medicine’s 2008 Annual Meeting in San Diego, Ca. It discusses the dangers and risks involved in hospital discharge. It provides information on the Project BOOST toolkit, designed to improve transitions of care which includes beneficial clinical tools and resources. Project BOOST was designed to help implement these tools which are provided via the Resource Room.
