American Heart Association (AHA) Patient Tools and Resources
The American Heart Association is an organization whose mission is to build healthier lives, free of cardiovascular diseases and stroke. It is active in developing guidelines and funding research for physicians in the area of cardiovascular disease. The AHA also has extensive patient education and information resources that are highly regarded and can be used to address modifiable risk factors for patients being discharged after an admission for ACS.

MedlinePlus® brings together authoritative information from NLM, the National Institutes of Health (NIH), and other government agencies and health-related organizations. Pre-formulated MEDLINE searches are included in MedlinePlus® and give easy access to medical journal articles.
MedlinePlus® also has extensive information about drugs, an illustrated medical encyclopedia, interactive patient tutorials, and latest health news. This resource can be used to provide specific information to patients at discharge or can be a resource for patients to explore after discharge on their own.

Hospital Compare
The Hospital Compare website was created by the United States Department of Health & Human Services (DHHS) Centers for Medicare & Medicaid Services (CMS), along with the Hospital Quality Alliance (HQA). The site allows patients to see data from hospitals process of care measures for heart attack, heart failure, pneumonia, or patients having surgery. This information is useful to help patients and caregivers make decisions about their healthcare. The site allows healthcare providers to identify areas for quality improvement.

The American Heart Association’s Get with the GuidelinesSM (GWTG)
In 2001, The American Heart Association (AHA) launched their Get With The GuidelinesSM (GWTG) hospital based quality improvement program, which is geared to helping hospitals be sure that the care they provide for patients with coronary artery disease, stroke, and heart failure is aligned with evidence-based treatments and therapies.

Case Management Society of America (CMSA)
The Case Management Society of America (CMSA) has several programs such as the Adherence Starts with Knowledge (ASK ®) that can help the healthcare team assess the patient’s adherence to medications and help patients make lifestyle changes.
The Adherence Starts with Knowledge (ASK ®) program which is a survey-based program designed to identify barriers to medication adherence and links to appropriate education materials.
