The University of Chicago Hospitals, Acute Coronary Syndrome Order Set
This order set, courtesy of Chad Whelan, MD was developed by a multidisciplinary team in an effort to standardize care for patients admitted with ACS at the University of Chicago. While the goal was to generally improve care, specific attention was paid to improving performance on the CMS core measures in the area of acute myocardial infarction. This order set is available at the University of Chicago on paper or via intranet as a downloadable PDF file.

Sutter Medical Center, Sacramento, California, Smoking Cessation Protocol Orders
The Sutter Medical Center website has made available ACS pre-printed orders and hospital forms via their website. These forms focus on the ACS core measures and include smoking cessation protocol orders.

Banner Baywood Heart Hospital and Medical Center, Acute Coronary Syndrome, Proposed Process of Care
Courtesy of Mark Starling, MD, Banner Heart Hospital CMO and member of the SHM Acute Coronary Syndrome (ACS) Advisory Board, this care map packet was designed for the general use of most cardiac patients but it may need to be adapted to meet the special needs of an individual patient as determined by the patients care giver. The purpose of this packet is to review and improve the process of care from ED to discharge for patients with an Acute Coronary Syndrome. Within the packet you will find ordersets for:

  • ED NSTEMI/UNSTABLE Angina High to Intermediate
