CMS/The Joint Commission, Specifications Manual for National Hospital Quality

The Specifications Manual for National Hospital Quality which can be accessed on www.jointcommission.org is the collaborative work of the Centers for Medicare & Medicaid Services and The Joint Commission. The specifications manual provides quick access to the hospital quality measures and tools to help promote and support both organizations common core measures.

Although currently, measures do not exist around discharge for the older adult, the measures that have been established by the Joint Commission and the Centers for Medicare and Medicaid may be applicable for the older adult patient if they are hospitalized for conditions such as pneumonia, ACS, Heart Failure, etc.
