Williams, Project BOOST Principal Investigator and Editor-in-Chief of the Journal of Hospital Medicine participated in this Webinar sponsored by the Commonwealth Reducing Rehospitalizations: A National Priority. Held on Thursday, April 2, at 2 p.m., E.T. Panelists addressed how reducing readmissions can improve patient outcomes and reduce waste and costs. They also outlined effective national, state and facility-level strategies to redesign care delivery and payment methods to foster reduction in rehospitalization rates.

Improving Quality and Safety During Transitions of Care
This presentation was given by Eric Coleman, MD, MPH, Jeffrey Greenwald, MD, Lakshmi Halasyamani, MD, and Mark V. Williams, MD members of Project BOOST (Better Outcomes for Older Adults in Safe Transitions) at the Society of Hospital Medicine’s 2008 Annual Meeting in San Diego, Ca. It discusses the dangers and risks involved in hospital discharge. It provides information on the Project BOOST toolkit, designed to improve transitions of care which includes beneficial clinical tools and resources.  Project BOOST was designed to help implement these tools which are provided via the Resource Room.
