The Joint Commission , CMS, AHA/ACC Core Measures

The Joint Commission continuously improves the safety and quality of care provided to the public through the provision of health care accreditation and related services that support performance improvement in health care organizations. The Joint Commission collaborates with Centers for Medicare and Medicaid Services (CMS) as well as the American Heart Association (AHA) and American College of Cardiology (ACC) with respect to the performance measures for patients with heart failure.
