Inpatient Goal Sheet
The Inpatient Goal Sheet is a multi-disciplinary rounding tool to help to set daily goals for the hospitalized patient. It may be useful for a variety of patients, and not only those with heart failure. The goal sheet creates an integrated home for team communication and addresses the myriad issues in preparing a patient for the discharge transition. It also serves as a reminder in case the patient is not progressing toward the identified goals, so that a re-assessment and review of the planned interventions can take place.

Conceptual Model for Teamwork
The Conceptual Model for Teamwork helps to frame the processes of care for a patient with heart failure. It can be used in the daily team-based care of the hospitalized patient and may be especially helpful in the care of complex patients who have multiple clinical issues and co-morbidities. The team leader/clinician should consider using the model when reviewing a patient’s daily care plan.
