University of Washington Medical Center Anticoagulation Services
The University of Washington Medical Center Anticoagulation Services site was created to aid with the management of anticoagulant therapy in four pharmacist-managed anticoagulation clinics as well as coordination of the use of antithrombotic agents in the inpatient setting. The site contains useful resources such as diagnostic algorithms for VTE, a treatment algorithm for VTE, and guidelines for the prevention of VTE. The site also provides guidelines for dosing and monitoring, guidelines for corrections of over-anticoagulation, protocols, patient education, and links to other anticoagulation sites.

The Joint Commissions Sentinel Event Alert-Issue # 41: Preventing Error Relating to Commonly Used Anticoagulants
In this Sentinel Event Alert, the Joint Commission outlines factors that contribute to medication errors involving anticoagulants, risk reduction strategies, existing Joint Commission requirements and other Joint Commission suggested actions.

VTE Safety Toolkit
The VTE Safety Toolkit created by the University of Washington funded by the Agency for Healthcare Research and Quality (AHRQ) provides a compilation of resources for prevention, diagnosis, and treatment of VTE.

Resources on Deep Vein Thrombosis and Pulmonary Embolism
The Surgeon General's Call to Action to Prevent Deep Vein Thrombosis and Pulmonary Embolism website has a webpage which is a compilation of several online resources around DVT. The page includes links to the Surgeon General’s Call to Action Workshop, the CDC, AHRQ, and Medline Plus.

Anticoagulant Toolkit
Reducing Adverse Drug Events & Potential Adverse Drug Events with Unfractionated Heparin, Low Molecular Weight Heparins and Warfarin. This toolkit developed by Purdue University PharmaTAP in collaboration with the Indiana Patient Safety Center (IPSC), Indiana Hospital Association (IHA) and VHA Central. The toolkit is designed to help hospitals meet the joint commission national public safety goal to reduce patient harm associated with anticoagulant therapy.
