Conceptual Model for Teamwork

The Conceptual Model for Teamwork, relevant to VTE prevention and many other quality improvement activities in the hospital, was developed by the SHM Heart Failure Initiative Team Communications Workgroup. View the Heart Failure Resource Room Project Team page for information on the workgroup members.

This Conceptual model helps to frame the processes of care for a patient with heart failure but the principles can be carried over to frame the processes of care for any hospitalized patient. It can be used in the daily team-based care of the hospitalized patient and may be especially helpful in the care of complex patients who have multiple clinical issues and co-morbidities. The team leader/clinician should consider using the model when reviewing a patient’s daily care plan.
