SHM QI Training

Glycemic Control Mentored Implementation (GCMI) Program

SHM will be accepting applications from 30 sites to participate in the Glycemic Control Mentored Implementation (GCMI) program. Mentors with QI and glycemic control expertise will work with each participating team to tackle site-specific issues using proven QI techniques. The range of issues will include: subcutaneous insulin protocols, transition from subcutaneous to infusion, infusion insulin, care coordination, improving follow up care, and having a good hypoglycemia management and prevention protocol.
Mentoring will occur during eight scheduled telephone calls offered over a 12 month period. Instruction will be organized around the Glycemic Control Implementation Guide, SHM’s step-by-step guide for developing a Quality Improvement Initiative to improve inpatient Glycemic Control.

  • Each of the selected sites will receive a year of mentoring from national experts to help optimize glycemic control at your hospital. There is no fee for participation. We encourage you to take advantage of this incredible opportunity. Selected sites will receive:
    • A year of mentoring provided via teleconferencing and e-mails to tailor the strategies outlined in the glycemic control implementation guide to the needs of your institution.
    • Project management tools.
    • Evaluation tools.
    • Access to networking opportunities to share successes and challenges with peer sites.
  • Start Date: April 2009 through November 2009.

Course Overview:
The 2009 Quality Pre-Course held at Hospital Medicine 2009 SHM’s Annual Meeting was re-designed to expose hospitalists of all experience levels to effective quality improvement projects. In an interactive format, multiple successful QI projects were profiled by the hospitalists who have led them. Participants had a chance to see key QI principles reappear in each project, while also gaining exposure to alternative strategies. The participant choose from one of three sessions: VTE, Glycemic Control, or Care Transitions.

Objectives:
At the conclusion of this activity, participants will be able to:

  • Describe how a hospitalist can communicate a vision to lead improvement in hospital care
  • Describe the power of standardization, especially when coupled with high reliability strategies
  • Describe features of a useful data system and how an effective multidisciplinary team can achieve specific performance goals
