Online CME

Successfully Managing Hospital Transitions in Diabetes Care
Sponsored by the Annenberg Center for Health Sciences, this CME activity is presented as a webcast. During the activity a panel will discuss best clinical practices to manage patients with diabetes and hyperglycemia as they transition into the hospital, from one unit to another within the hospital, and onto discharge.

Upon completion of this activity, participants should be better able to:

  • Describe the growing problem of diabetes and hyperglycemia and their impact on patients entering the hospital
  • Outline assessment strategies and initial inpatient glycemic management for the newly hospitalized patient
  • Discuss the use of insulin and nutritional support for patients with diabetes and hyperglycemia within the hospital to ensure safe transitions from criticalcare to other inpatient settings
  • Determine educational and discharge needs of patients and their families as well as improve communication with outpatient care providers

Improving Glycemic Control in the Inpatient Setting: An Interdisciplinary Team Approach
This CME activity will address successful methods for improving glycemic control in hospitalized patients, based on national guidelines and performance measures. Strategies for ensuring safe and effective administration and monitoring of insulin therapy in the hospital setting will be discussed using patient case studies. Additionally, the faculty will discuss interdisciplinary approaches to developing and implementing protocols for glycemic control in the hospital setting.

At the conclusion of the activity, participants should be able to

  • Summarize the importance of strict inpatient glycemic control to patient outcomes in hospitalized patients.
  • List and describe national guidelines and performance measures for appropriate glycemic control in hospitalized patients.
  • Propose a hospital-wide systematic plan for developing and implementing strict glycemic control policies and protocols and ensuring safety in insulin therapy in the hospital setting.

This activity is designated for a maximum of 2.0 AMA PRA Category 1 CreditTM.
