Data Collection Tools
Yale Center for Medical Informatics Glucometrics-Measuring Inpatient Glycemic Control
The Glucometrics Web site allows downloads of deidentified glucose data, with which it can automatically prepare reports on glucose control. Currently analysis of the following measures can be included in the reports:
- General results
- Number of included and number of excluded patients.
- Average number of glucose readings per patient for the entire hospitalization (ie, patient-stay).
- Average number of readings per patient-day.
- Glucose reading
- Average of all glucose readings from all patients.
- Percentage of glucose readings within specified ranges: <60, >300, and 80–139 mg/dL.
- Patient stay
- Average glucose per patient stay.
- Percentage of patients whose average glucose is in the target range (80–139 mg/dL).
- Percentage patients with any result < 60 or > 300 mg/dL.
- Patient-day
- Average glucose per patient-day, ie, patient-day weighted mean.
- Percentage of patient-days with average glucose in the target range (80–139 mg/dL).
- Percentage of patient-days with any result < 60 or > 300 mg/dL.

The clinical tools were compiled by the Glycemic Control Resource Room Project Team to provide teams developing interventions to improve inpatient care with “real world” examples of what other institutions use. The tools have been implemented in a variety of clinical settings and include order sets, guidelines, and protocols. The resulting library of institution specific tools does not imply endorsement by the Glycemic Control Resource Room Project Team or SHM.
