SHM Glycemic Control Mentored Implementation (GCMI)
The GCMI Program
Mentors with QI and glycemic control expertise will work with each participating team to tackle site-specific issues using proven QI techniques. The range of issues will include: subcutaneous insulin protocols, transition from subcutaneous to infusion, infusion insulin, care coordination, improving follow up care, and having a good hypoglycemia management and prevention protocol.
Mentoring will occur during eight scheduled telephone calls offered over a 12 month period. Instruction will be organized around the Glycemic Control Implementation Guide, SHM's step-by-step guide for developing a GCMI program.
Each of the selected sites will receive a year of mentoring from national experts to help optimize glycemic control at your hospital. There is no fee for participation. We encourage you to take advantage of this incredible opportunity. Selected sites will receive:
- The GCMI Toolkit and a year of mentoring provided via teleconferencing and e-mails to tailor the toolkit to the needs of your institution, implement it and evaluate its impact.
- Project management tools.
- Evaluation tools.
- Access to networking opportunities to share successes and challenges with peer sites.

SHM’s mentored implementation programs- BOOST, GCMI and VTE-PC- are now approved through ABIM for 20 points towards the Self-Evaluation of Practice Performance requirement of Maintenance of Certification (MOC). Click here for more information.
Goal and outcomes: The goal of GCMI is to support the development and implementation of glycemic control programs at multiple member sites. By improving glycemic control, GCMI aims to support sites in optimizing the care of inpatients with hyperglycemia and diabetes and in preventing hypoglycemia.
What is Mentored Implementation?
APPROACH:
- Create a national consensus for best practices. GCMI's advisory board includes representatives from American College of Physicians, American Diabetes Association, American Association of Clinical Endocrinologists, American Society of Consultant Pharmacists, Case Management Society of America and the Society of Hospital Medicine.
- Create resources to implement best practices. GCMI created the Glycemic Control Resource Room for use by quality improvement teams. Components of the Resource Room include:
- Provide technical support. GCMI offers technical support including the year-long mentoring program sponsored by sanofi-aventis US, LLC. The selection of mentors and formulary choices are in dependent of sponsorship.

eQUIPS Glycemic Control Initative
SHM's Electronic Quality Improvement Programs (eQUIPS) are web-based toolkits designed to help jumpstart your quality improvement and patient safety activities in three topic areas: Care Transitions, Glycemic Control and VTE Prevention. The eQUIPS Glycemic Control Program focuses on optimizing the care of inpatients with hyperglycemia and diabetes. An implementation guide along with SHM's resource room—a virtual library of resources— addresses various issues that range from subcutaneous insulin protocols to care coordination and having good hypoglycemia management.
