The Society of Hospital Medicine’s (SHM’s) Fellows program is rooted in the Society’s Core Competencies in Hospital Medicine. Those who earn the Fellow in Hospital Medicine (FHM), Senior Fellow in Hospital Medicine (SFHM) or Master in Hospital Medicine (MHM) designations have demonstrated their commitment to hospital medicine, system change and quality improvement principles. INSTRUCTIONS FOR FELLOW AND SENIOR FELLOW IN HOSPITAL MEDICINE APPLICANTS

 

  1. Solicit two letters of recommendation from active members of SHM who have been in good standing for at least two years.
  2. Submit a one-time processing fee of $150 for Fellows and $175 for Senior Fellows. This fee must be submitted with your application and is non-refundable. (Applications will not be processed until the fee has been received.)

IMPORTANT DATES AND REQUIREMENTS SUMMARY

  1. Application review is contingent on compliance.
  2. Deadline for completed applications is November 30, 2017.
  3. Results will be announced on or before December 31, 2017.
  4. EARLY DECISION is available for applications completed on or before September 15, 2017.  These applicants will be notified of our decision on or before October 27, 2017.
  5. Application is NOT COMPLETE until endorsements are submitted and payment is submitted.
  6. It is the responsibility of the APPLICANT to confirm that endorsements are submitted by the deadline.
  7. If declined, the application will NOT transfer to the following year.

FELLOW IN HOSPITAL MEDICINE - PHYSICIAN ELIGIBILITY AND INSTRUCTIONS


ELIGIBILITY REQUIREMENTS FOR ADVANCEMENT TO FELLOW IN HOSPITAL MEDICINE


To achieve the Fellow in Hospital Medicine designation, applicants must successfully:

  1. Fulfill all of the Necessary Eligibility Criteria detailed below.
  2. Demonstrate experience in hospital medicine leadership, teamwork, and quality improvement by achieving the requisite number of points from the Core Hospital Medicine Competency Criteria detailed in the application.

 

NECESSARY ELIGIBILITY CRITERIA


Eligible Fellow applicants must fulfill all of the following:

  1. MD, DO, or equivalent foreign degree and board certification in a specialty recognized by the ABMS.
  2. Time spent on clinical and non-clinical work that is devoted to the practice of hospital medicine must comprise at least 50% of total professional activity.
  3. Minimum of five years as practicing hospitalist (Fellowship in hospital medicine can be claimed to meet this requirement).
  4. Minimum of three years as a member of SHM.
  5. Current SHM membership is in good standing, and payment of dues is up to date.
  6. Registration and attendance at a minimum of one SHM Annual Meeting plus one of the following (Note: Registration for future meetings cannot be claimed to satisfy this requirement):
  7. • One additional SHM Annual Meeting
    • One SHM Leadership Academy
    • One Regional Meeting (i.e., San Francisco, Southern, Midwest)
    • One National Pediatric Hospital Medicine Meeting (tri-sponsored by SHM, AAP, and APA)
    • One Canadian Hospital Medicine Society National Meeting

  8. Endorsement via letters of recommendation by two active SHM members who have been in good standing for at least two years.
  9. No history of professional disciplinary action resulting in suspension or revocation of credentials or license within the past five years.

ELIGIBILITY REQUIREMENTS FOR ADVANCEMENT TO FELLOW IN HOSPITAL MEDICINE

 

To achieve the Fellow in Hospital Medicine designation, applicants must successfully:

  1. Fulfill all of the Necessary Eligibility Criteria detailed below.
  2. Demonstrate experience in hospital medicine leadership, teamwork, and quality improvement by achieving the requisite number of points from the Core Hospital Medicine Competency Criteria detailed in the application.

  3. Minimum of five years in a Hospital Medicine setting.
  4. Minimum of three years as a member of SHM.
  5. Current SHM membership is in good standing, and payment of dues is up to date.
  6. Registration and attendance at a minimum of one SHM Annual Meeting plus one of the following (Note: Registration for future meetings cannot be claimed to satisfy this requirement):

  4. Minimum of three years as a member of SHM.
  5. Current SHM membership is in good standing, and payment of dues is up to date.
  6. Registration and attendance at a minimum of one SHM Annual Meeting plus one of the following (Note: Registration for future meetings cannot be claimed to satisfy this requirement):
  7. • One additional SHM Annual Meeting
    • One SHM Leadership Academy
    • One Regional Meeting (i.e., San Francisco, Southern, Midwest)
