SHM VTE Prevention Collaborative Privacy Statement
The Society of Hospital Medicine (SHM) is committed to respecting the privacy of all applicants to the VTE Prevention Collaborative (VTE PC) program. We recognize that when you choose to provide us with information about yourself and your institution, you trust us to protect your privacy.
By applying to the VTE PC program, you consent to the collection and use of your information by SHM and VTE PC program staff, as described below.
Types of Information Requested:
SHM will collect information such as name, title, mailing address, telephone number, and email address from applicants wishing to participate in the VTE PC program. We will also ask applicants about quality improvement (QI) training they may have completed and current or previous involvement in QI projects. In addition, we will collect information about the hospital at which the VTE prevention effort will be implemented, as well as information about the structure and functioning of planned, active or previous VTE prevention efforts at that site.
For applicants who are accepted into the program, we will collect information about the progress of efforts to launch or improve local VTE prevention programs, and on the structure and functioning of those efforts. All data will be both password protected and encrypted
How Your Information Will Be Used:
VTE PC staff will use the provided contact information to communicate with applicants and enroll individuals in the program. Descriptive information about applicants and affiliated institutions will be used when considering mentor/consultant assignments and will help mentors/consultants frame instruction.
SHM and VTE PC staff will not disclose to third parties any information gathered from you without first obtaining your express consent to do so.
Information collected in applications and other program forms may be de-identified and used in the aggregate to help us evaluate the effectiveness of the VTE PC program or when drafting publications describing the initiative.
