New Applicants

Step 1: Read the following information and requirements:
SHM VTE Prevention Collaborative mentors, consultants and staff will use the information requested in the application to develop an understanding of the status of your planned or active VTE prevention program. The application should take less than 30 minutes to complete. In addition, successful applicants will be asked to supply a signature of support from a senior hospital, clinical department or medical group leader. A document your hospital/department/group leader can sign will be sent to you once your application has been processed.
You will be able to save your application at any point and log back in to complete it at your convenience.
You will receive a communication regarding the status of your application within 14 days of receipt.
