Project BOOST® Mentored Implementation Program

Project BOOST® Mentored Implementation Program is a national initiative led by the Society of Hospital Medicine (SHM) to improve the care of patients as they transition from hospital to home.

The Project BOOST® Mentored Implementation Program is a yearlong initiative wherein hospitals receive expert mentoring and peer support to aid in improving the care of patients as they transition from hospital to home. BOOST mentors help hospital teams to map current processes and create and implement action plans for organizational change. BOOST provides a suite of evidence-based clinical interventions that can be easily adapted and integrated into each unique hospital environment.

  • Improve facility patient satisfaction and HCAHPS scores

BOOST Mentored Implementation ComponentsClick to expand

Through the Project BOOST Mentored Implementation Program, sites will receive:

  • A Comprehensive Intervention developed by a panel of nationally recognized experts based on the best available evidence
  • A Comprehensive Implementation Guide providing step-by-step instructions and project management tools, such as the TeachBack Training Curriculum, to help interdisciplinary teams redesign workflow and plan, implement and evaluate the intervention
  • Longitudinal Technical Assistance providing face-to-face training and a year of expert mentoring and coaching to implement Project BOOST interventions that build a culture that supports safe and complete transitions; the program provides a “Train the Trainer” DVD and a curriculum for nurses and case managers on using the TeachBack process as well as webinars targeting the educational needs of other team members including administrators, data analysts, physicians, nurses and others
  • The BOOST Collaboration allowing sites to communicate with and learn from each other via the BOOST Listserv, BOOST Community site and quarterly all-site teleconferences and webinars
  • The BOOST Data Center, an online resource center, allowing sites to store and benchmark data against control units and other sites and generate reports
